What California employers need to know

Who's eligible?

Any California business with 1 or more W-2 employees can offer group coverage. Most carriers require at least 2 enrolled employees. Sole proprietors with no employees typically use individual plans instead.

What does it cost?

Premiums depend on your group's size, employee ages, region, and plan design. In 2026, a Gold-tier plan for a 10-person group in the Bay Area typically runs $600–$900 per employee per month. Employers must contribute at least 50% of the employee-only premium.

When can you enroll?

New groups can start coverage any month of the year. Renewals happen annually on your plan's effective date. We recommend shopping the market 60–90 days before renewal to compare options and negotiate.

HMO, PPO, or HDHP?

HMOs (like Kaiser) cost less but limit provider choice. PPOs offer more flexibility and out-of-network coverage. HDHPs pair with HSAs for tax-advantaged savings. Most groups benefit from offering at least two options.

Do you have to offer it?

California employers with fewer than 50 full-time equivalent employees are not required to offer health insurance under the ACA. But offering coverage helps attract and retain talent — and the tax benefits can be significant.

Is a broker free?
